دعوني أبدأ بقصتي لقد هشمت غضروف الركبة المشترك اثناء ممارستي كرة القدم في الجامعة ومن ثم حصل لدي تمزق في الاربطة الموجودة في الركبة ومن ثم اصبت بالتهاب المفاصل - في الركبة - واعتقد ان العديد من الجمهور هنا يعانون من نفس المشكلة وبالمناسبة لقد تزوجت من إمرأة لديها نفس القصة تماما وقد دفعني هذا لكي أغدو جراح عظام ولانني اهتممت كثيرا بحلول تلك المشاكل فإنني اليوم امارس الرياضة بكل انواعها وبشتى الطرق بداية دعوني اعرض لكم هذا الفيديو لكي اعطيكم نظرة عامة عما احاول ان اشرحه لكم
So let me just start with my story. So I tore my knee joint meniscus cartilage playing soccer in college. Then I went on to tear my ACL, the ligament in my knee, and then developed an arthritic knee. And I'm sure that many of you in this audience have that same story, and, by the way, I married a woman who has exactly the same story. So this motivated me to become an orthopedic surgeon and to see if I couldn't focus on solutions for those problems that would keep me playing sports and not limit me. So with that, let me just show you a quick video to get you in the mood of what we're trying to explain.
المذيع : نحن جميعا نعي خطر مرض السرطان ولكن هناك مرضٌ آخر وهو يؤثر على عدد اكبر من مرضى السرطان .. انه التهاب المفاصل صحيح ان السرطان قد يقتلك .. ولكن عندما ننظر الى الارقام فان نجد ان التهاب المفاصل يدمر حياة الكثيرين منا ولنفترض انك ستعيش عمراً مديد ... فان هنالك احتمالا 50% بأن تصاب بالتهاب المفاصل وليس فقط التقدم بالعمر الذي يصيبك به انما الاصابات الشائعة التي نتعرض لها يمكنها ان تعرضنا لعقود من الالآم المبرحة حتى تنطحن المفاصل تماما وتتحطم " حرفيا" ! ولاننا نحتاج الى حل ما . .فقد لجأنا الى الهندسة لكي نصمم عناصر اصطناعية بديلة عن تلك التي تعطب في جسدنا ولكن وضمن صيحة العصر الحالية فاننا نطمح لحل بيولوجي أكثر طبيعية .. واكثر ملائمة لنا ولكي نقيم تلك الطريقة الجديدة علينا ان نسأل انفسنا ... ماذا لو كانت كل البدائل المطلوبة لجسدنا موجودة بالفعل في الطبيعة او داخل الخلايا الجذعية هذا هو مجال البدائل البيولوجية حيث تستبدل العناصر المعطوبة بأخرى طبيعية
Narrator: We are all aware of the risk of cancer, but there's another disease that's destined to affect even more of us: arthritis. Cancer may kill you, but when you look at the numbers, arthritis ruins more lives. Assuming you live a long life, there's a 50 percent chance you'll develop arthritis. Let's consider an alternative path. What if all the replacements our bodies need already exist in nature, or within our own stem cells? This is the field of biologic replacements, where we replace worn-out parts with new, natural ones.
كيفين ستون : لذا تكمن مهمتنا بطرح السؤال .. كيف يمكننا معالجة التهاب المفاصل " بيولوجيا " دعونا نتحدث عما قمت به لنفسي ولزوجتي ولعدة مئات من المرضى الذين عالجت اولا بالنسبة لزوجتي وايضا بالنسبة للمرضى الذين اعالجهم والذي معظمهم اعمارهم بين 40-80 عام .. او 70 عام فهم يأتون إلي ويقولون دكتور .. هل يمكن ان اضع ممتص صدمات في ركبتي بدلا من ان الجأ الى عملية تبديل المفصل ! عندما عالجت زوجتي قمت بوضع غضروف بشري من متبرع في ذلك الفراغ عند المفصل لاستبدال المعطوب ومن ثم استبدال الاربطة المتمزقة برباط من متبرع انساني لكي تستقر الركبة ومن ثم من اجل العظام المهترئة على السطح قمنا بعمل غلاف من خلايا قمنا بتصنيعها عام 1991 حيث تنمو لاحقا على سطح المفصل الغضروفي وتعطي انسيابية للمفصل هناك وفي الصورة ترون ركبة زوجتي المعطوبة وفي الصورة المقابلة تمارس التنزه الجبلي منذ اربعة اشهر في آسبن وحالتها ممتازة وقد نجحت تلك العملية .. ونجحت حتما لباقي المرضى فالفتاة التي في الفيديو هي واحدة من المتزلجات في اسبن وبعد تسعة اشهر من تهشيم ركبها التي ترون في الصورة امامكم قمنا بعملية " ترقيع " لتلك المنطقة لكي ينمو السطح بيولوجيا ويترمم بصورة تامة
Kevin Stone: And so, the mission is: how do I treat these things biologically? And let's talk about both what I did for my wife, and what I've done for hundreds of other patients. First thing for my wife, and the most common thing I hear from my patients, particularly in the 40- to 80-year-old age group, 70-year-old age group, is they come in and say, "Hey, Doc, isn't there just a shock absorber you can put in my knee? I'm not ready for joint replacement." And so for her, I put in a human meniscus allograft donor right into that [knee] joint space. And [the allograft] replaces [the missing meniscus]. And then for that unstable ligament, we put in a human donor ligament to stabilize the knee. And then for the damaged arthritis on the surface, we did a stem cell paste graft, which we designed in 1991, to regrow that articular cartilage surface and give it back a smooth surface there. So here's my wife's bad knee on the left, and her just hiking now four months later in Aspen, and doing well. And it works, not just for my wife, but certainly for other patients. The girl on the video, Jen Hudak, just won the Superpipe in Aspen just nine months after having destroyed her knee, as you see in the other image -- and having a paste graft to that knee. And so we can regrow these surfaces biologically.
والآن ومع كل هذا النجاح يبقى هذا الحل قاصرا .. وسوف تتسائلون لماذا .. السبب هو انه لانملك العدد الكافي من المتبرعين فلا يوجد متبرعين شباب حالتهم الصحية جيدة يقعون كل يوم من على دراجاتهم الالية ووتوفاهم المنية ..ومن ثم يغدون متبرعين للاعضاء كما ان ذلك الغلاف الذي يستخدم للترقع مرتفع الكلفة جدا لذا هذا الحل لا يعدُ حلاً ولا يمكن اعتباره حلا بيولوجيا عالمي يمكن استخدامه على نطاق واسع ولكن الحل هو الانسجة الحيوانية لانها متوفرة بكثرة ورخصية الثمن ويمكن ان تحصل عليها من الحيوانات الشابة الصحية ... اي يمكن الحصول على انسجة صحية ولكن الذي يقف عائقا هنا هو الجهاز المناعي و يمكن العائق تحديدا في جزيء حانمة هو الجزء من المستضد الذي يتم التعرف عليه بواسطة الجهاز المناعي يُسمى الجلاكتوسيل او حانمة الجلاكتوسيل لذا اذا اردنا ان نستخدم الاغشية الحيوانية في البشر علينا اولا معرفة طريقة يمكننا فيها من انتزاع تلك " الحانمة "
So with all this success, why isn't that good enough, you might ask. Well the reason is because there's not enough donor cycles. There's not enough young, healthy people falling off their motorcycle and donating that tissue to us. And the tissue's very expensive. And so that's not going to be a solution that's going to get us global with biologic tissue. But the solution is animal tissue because it's plentiful, it's cheap, you can get it from young, healthy tissues, but the barrier is immunology. And the specific barrier is a specific epitope called the galactosyl, or gal epitope. So if we're going to transplant animal tissues to people, we have to figure out a way to get rid of that epitope.
وتبدأ قصتي في العمل على الاغشية الحيوانية منذ عام 1984 حيث بدأت اولا مع عظم العرقوب لاحدى الابقار حيث اخذنا عظم العرقوب ذلك والذي هو من نوع ( كولاجين - 1 ) وقمنا بنزع المستضدات منه بواسطة غسله بحمض ومواد منظفة وشكلنا منه نموذجا تشكيليا ومن ثم نأخذ ذلك النموذج ونضعه ضمن الغضروف المعطوب لكي ينمو داخل ركبة المريض وقد قمنا بتلك العملية على 4000 حالة حول العالم وهي طريقة معترف بها من منظمة الصحة والدواء الامريكية .. وحول العالم ايضا لاعادة بناء غضروف جديد وهذه طريقة ممتازة للغضاريف المهشمة ولكن ماذا يحدث عندما احتاج الى وتر غضروفي فانا لايمكني ان اضع ما اريد في الخلاط لكي احصل في النهاية على غضروف في هذه الحالة علينا ان نصمم ذلك الوتر الغضروفي - وقد قمت انا ويوري جلالي وتوم تركي - بغسيل انزيمي لكي نقوم بازالة تلك المستضدات -الحانمة - باستخدم الانزيم المناسب وقد اطلقنا اسم - تعرية الغال - على تلك التقنية اي ما نقوم به هو تحويل ذلك الغشاء الحيواني الى بشري عن طريق تعرية الغال نحن نجعله بشري اكثر ومن ثم يمكننا وضعه في ركبة المريض والذي قمنا به اليوم هو اننا أخذنا اوتار من حيوان الخنزير يكون شابا وصحيا وناخذ اوتار كبيرة ووضعناها في 10 مرضى - بموافقة هيئة الغذاء والدواء الامريكية وواحد من الذين عالجناهم حصل على ثلاث بطولات في مسابقة تزلج الهضبة في كندا على قدمه " التي استخدم فيها اغشية الخنزير " لذا نحن نعي اننا يمكننا القيام بذلك وهنالك تجارب حول العالم عن هذا الامر ستقام قريباً
So my story in working with animal tissues starts in 1984. And I started first with cow Achilles tendon, where we would take the cow Achilles tendon, which is type-I collagen, strip it of its antigens by degrading it with an acid and detergent wash and forming it into a regeneration template. We would then take that regeneration template and insert it into the missing meniscus cartilage to regrow that in a patient's knee. We've now done that procedure, and it's been done worldwide in over 4,000 cases, so it's an FDA-approved and worldwide-accepted way to regrow the meniscus. And that's great when I can degrade the tissue. But what happens for your ligament when I need an intact ligament? I can't grind it up in a blender. So in that case, I have to design -- and we designed with Uri Galili and Tom Turek -- an enzyme wash to wash away, or strip, those galactosyl epitopes with a specific enzyme. And we call that a "gal stripping" technique. What we do is humanize the tissue. It's by gal stripping that tissue we humanize it (Laughter), and then we can put it back into a patient's knee. And we've done that. Now we've taken pig ligament -- young, healthy, big tissue, put it into 10 patients in an FDA-approved trial -- and then one of our patients went on to have three Canadian Masters Downhill championships -- on his "pig-lig," as he calls it. So we know it can work. And there's a wide clinical trial of this tissue now pending.
ولكن ماهي الخطوة المقبلة ماذا عن الحصول على ركبة بيولوجية كاملة - اي القيام باستبدال كامل - ليس فحسب الاجزاء كيف سنقوم بالقيام بثورة بعالم استبدال الاطراف هذا الذي ننوي القيام به سوف نأخذ جزىء معين من غضروف خنزير صحيح شاب ونعريه من مستضداته ومن ثم نضع عليه الخلايا الجذعية الخاصة بالمريض ومن ثم نقوم بوضعه على سطح الركبة ونلصقه هناك وسوف يشفى السطح لانه سنمو سطح جديد على سطح الركبة القديم وهذه هي الطريقة التي نريد القيام بها اليوم وسوف نقوم بإعادة بناء اجزاء ركبة المريض وسنقوم بتغطية جديدة لسطح الركبة القديم
So what about the next step? What about getting to a total biologic knee replacement, not just the parts? How are we going to revolutionize artificial joint replacement? Well here's how we're going to do it. So what we're going to do is take an articular cartilage from a young, healthy pig, strip it of its antigens, load it with your stem cells, then put it back on to that arthritic surface in your knee, tack it on there, have you heal that surface and then create a new biologic surface for your knee. So that's our biologic approach right now. We're going to rebuild your knee with the parts. We're going to resurface it with a completely new surface.
ولكن لدينا مميزات اخرى من مملكة الحيوان ميزة موجودة من 400 مليون سنة من التكيف والتطور حيث يمكننا الاستفادة من تلك المميزات اي يمكننا استخدام انسجة اثخن واقوى واكثر شبابا لكي نستبدل تلك التي لديك في ركبتك او تلك التي تملك في عمر 40 او 50 او 60 ويمكننا تنميتها خارج الجسم ويمكننا ان ننزع تلك الانسجة من الحيوانات بصورة رخصية جدا ويمكننا الحصول على عمليات استبدال ركب بصورة بيولوجية ناجحة ويمكننا نشرها حول العالم
But we have other advantages from the animal kingdom. There's a benefit of 400 million years of ambulation. We can harness those benefits. We can use thicker, younger, better tissues than you might have injured in your knee, or that you might have when you're 40, 50 or 60. We can do it as an outpatient procedure. We can strip that tissue very economically, and so this is how we can get biologic knee replacement to go global.
