As a child, I discovered that creativity is the key to cross from dreams to reality. I learned this from my grandmother, Dr. Ruth Tichauer, a Jewish refugee that settled in the heart of the Andes. That is how I grew up: encouraged to see beyond any limitation. So part of my education included helping her in remote, indigenous communities. I cherish those memories, because they helped me to understand life outside the city, a life with a lot of possibilities, without barriers, as language or culture.
Em criança, descobri que a criatividade é a chave para passar dos sonhos à realidade. Aprendi isto com a minha avó, a Dra. Ruth Tichauer, uma refugiada judia que se instalou no coração dos Andes. Foi assim que cresci: encorajado a ver para além das limitações. Parte da minha educação incluiu ajudá-la em comunidades indígenas e longínquas. Acarinho estas lembranças, porque me ajudaram a entender a vida fora da cidade, uma vida com imensas possibilidades, sem barreiras como a língua ou a cultura.
During those trips, my grandmother used to recite a Kipling poem: "Something hidden. Go and find it. Go and look behind the Mountains. Something lost behind the mountains. Lost and waiting for you. Go!"

In the coming years, I became a medical student. One of every hundred children born worldwide has some kind of heart disease. There's a part of this problem I think I can solve -- the part of this problem I have spent my life working on.
Nos anos seguintes, fui estudar medicina. Uma em cada cem crianças no mundo nascem com uma doença cardíaca. Penso poder resolver uma parte deste problema — passei a vida a trabalhar em parte desse problema.
The problem starts during pregnancy. The fetus needs to survive inside the mother. Survival depends on communication between the systemic and the pulmonary blood. At the moment of birth, this communication needs to stop. If it doesn't close, the baby has a hole in the heart. It is caused by prematurity and genetic conditions. But what we know today is that a lack of oxygen is also one of the causes. As you can see in the chart, the frequency of this kind of hole dramatically increases with altitude.
O problema surge durante a gravidez. O feto tem de sobreviver dentro da mãe. A sobrevivência depende da comunicação entre o sangue sistémico e o sangue pulmonar. Na altura do nascimento, esta comunicação tem de parar. Se tal não acontecer, o bebé fica com um buraco no coração. A causa pode ser a prematuridade ou condições genéticas Mas hoje sabemos que a falta de oxigénio também pode ser a causa. Como podem ver no gráfico, a frequência deste tipo de buraco aumenta drasticamente com a altitude.

When you look at patients with this condition, they seem desperate to breathe. To close the hole, major surgery used to be the only solution.
Quando observamos doentes nesta situação médica, os bebés tentam respirar desesperados. A cirurgia era a única solução para fechar o buraco.
One night, my friend Malte, were camping in the Amazon region. The only thing that would not burn in the fire was a green avocado branch. Then came a moment of inspiration. So we used the branch as a mold for our first invention. The holes in children's hearts can be closed with it. A coil is a piece of wire wrapped onto itself. It maybe doesn't look so fancy to you now, but that was our first successful attempt to create a device for this major problem. In this video, we can see how a very tiny catheter takes the coil to the heart. The coil then closes the hole. After that moment of inspiration, there came a very long time of effort developing a prototype. In vitro and in vivo studies took thousands of hours of work in the lab. The coil, if it works, can save lives.
Uma noite, o meu amigo Malte e eu, estávamos a acampar na região da Amazónia. A única coisa que não ardia na fogueira era um ramo verde de abacateiro Surgiu então um momento de inspiração. Usámos o ramo como um modelo para a nossa primeira invenção. Os buracos nos corações das crianças podiam ser fechados com aquilo. Uma bobina é um bocado de fio enrolado em si mesmo. Pode não parecer muito sofisticado agora, mas foi a nossa primeira tentativa bem sucedida para criar um dispositivo para este grande problema. Neste vídeo, podemos ver como um cateter muito fino leva a bobina até ao coração. Depois, a bobina fecha o buraco. Depois desse momento de inspiração, seguiu-se um longo período de trabalho para desenvolver um protótipo. Os estudos "in vitro" e "in vivo" levaram milhares de horas no laboratório. A bobina, se funcionar, pode salvar vidas.
I returned from Germany to Bolivia, thinking that wherever we go, we have the opportunity to make a difference. With my wife and partner, Dr. Alexandra Heath, we started to see patients. After successfully treating patients with our coil, we felt really enthusiastic. But we live in a place that is 12,000 feet high. And, the patients there need a special device to solve their heart condition. The hole in altitude patients is different, because the orifice between the arteries is larger. Most patients cannot afford to be treated on time, and they die. The first coil could successfully treat only half of the patients in Bolivia. The search started again. We went back to the drawing board.
Voltei da Alemanha para a Bolívia, pensando que, onde quer que fôssemos, teríamos a oportunidade de fazer a diferença. Com a minha mulher e colega, Dra. Alexandra Heath, começámos a ver doentes. Depois de tratamentos bem sucedidos usando a nossa bobina, ficámos muito entusiasmados. Mas vivemos num lugar a mais de 3600 metros de altitude. Para estes doentes, é necessário um aparelho especial para resolver a situação do coração. O buraco em doentes em grandes altitudes é diferente, porque o orifício entre as artérias é maior. A maioria dos doentes não tem meios para poder ser tratado a tempo, e morrem. A primeira bobina podia tratar de forma bem sucedida apenas metade dos doentes na Bolívia. A investigação recomeçou. Voltámos ao princípio.
After many trials, and with the help of my grandmother's indigenous friends in the mountains, we obtained a new device. For centuries, indigenous women told stories by weaving complex patterns on looms, and an unexpected skill helped us for the new device. We take this traditional method of weaving and make a design made by a smart material that records shape. It seems this time, the weaving allows us to create a seamless device that doesn't rust because it's made of only one piece. It can change by itself into very complex structures by a procedure that took decades to develop.
Depois de muitas tentativas, e com a ajuda dos amigos indígenas da minha avó, nas montanhas, criámos um novo dispositivo. Durante séculos, as mulheres indígenas contaram histórias tecendo padrões complexos em teares, e esta técnica inesperada ajudou-nos no novo dispositivo. Pegámos neste método tradicional de tecelagem e criámos um desenho feito de um material inteligente que memoriza formas. Desta vez, parecia que a tecelagem nos permitia criar um dispositivo sem costuras que não enferrujava porque era feito de uma só peça. Pode transformar-se, sozinho, em estruturas muito complexas por um processo que levou décadas a aperfeiçoar.
As you can see, the device enters the body through the natural channels. Doctors have only to close the catheter through the hole. Our device expands, places itself and closes the hole. We have this beautiful delivery system that is so simple to use because it works by itself. No open surgery was necessary.
Como veem, o dispositivo entra no corpo pelos canais naturais. Os médicos só têm que introduzir o cateter através do buraco. O nosso dispositivo expande-se, posiciona-se e fecha o buraco. Temos este belo sistema de execução que é muito simples de usar porque funciona por si mesmo. Não é necessária uma cirurgia aberta.

As you can see, once the device is in place, the patient is 100 percent healed. From start to finish, the whole procedure takes only 30 minutes. That's very rewarding from the medical and human point of view. We are so proud that some of our former patients are part of our team -- a team, thanks to added close interaction with patients that work with us. Together, we have only one idea: the best solutions need to be simple. We lost the fear of creating something new.
Como veem, depois de o dispositivo estar no seu lugar, o doente está completamente curado. Do início até ao fim, todo o processo demora apenas 30 minutos. É muito gratificante, do ponto de vista médico e humano. Sentimo-nos orgulhosos porque alguns dos nossos antigos doentes fazem parte da nossa equipa — uma equipa, graças à estreita interação com doentes que trabalham connosco. Em conjunto, só temos uma ideia: a melhor solução tem que ser simples. Perdemos o medo de criar uma coisa nova.
The path, it's not easy. Many obstacles arise all the time. But we receive strength from our patients. Their resilience and courage inspire our creativity. Our goal is to make sure that no child is left behind, not because of cost or access.
O caminho não é fácil. Estão sempre a surgir muitos obstáculos. Mas os nossos doentes dão-nos forças. A sua teimosia e coragem inspiram a nossa criatividade. O nosso objetivo é garantir que nenhuma criança é negligenciada, por causa do custo ou do acesso.
So we have to start a foundation with a one-to-one model. We will give one device for free to make sure that every child is treated. We are in many countries now, but we need to be everywhere. This whole thing began with one impossible idea as will continue it, really: No child is left behind.
Por isso, temos que iniciar uma fundação com um modelo individualizado. Vamos dar um dispositivo, grátis, para garantir que todas as crianças são tratadas. Estamos hoje em muitos países, mas temos que estar em todo o lado. Tudo isto começou com uma ideia impossível, e assim continuaremos: Nenhuma criança será negligenciada.
